It’s now 2026, and if you’ve looked at your new insurance cards lately, you’ve probably noticed some changes. Between the new "NYRx" rules for Medicaid and the big shifts in Medicare Part D, it’s a lot to keep track of. My goal is to make sure that no matter what card is in your wallet, you’re getting every penny’s worth of the benefits you’re entitled to.
The Big Win for Medicare in 2026
For my neighbors on Medicare, there is actually some really good news this year. For the first time, your out-of-pocket costs for prescription drugs are capped at $2,100 for the entire year.
In years past, I’ve had to have heartbreaking conversations with seniors who hit the "donut hole" and suddenly saw their medication costs jump to hundreds of dollars a month. That’s over. Once you spend $2,100 on your covered meds, your plan pays 100% for the rest of 2026.
Also, if you’re taking insulin, remember that your copay is still capped at $35 for a 30-day supply. You shouldn't be paying a penny more, and if you are, come see me, we might need to check if your brand is on your plan's "preferred" list.
Navigating Medicaid (NYRx)
If you’re on Medicaid, you’re likely using the NYRx program. One thing I always remind people is that you have more choice than you think. You don't have to go to a giant chain where they don't know your name just because you have a Medicaid card.
At CityMeds, we accept NYRx and can help you navigate things like "Prior Authorizations." If your doctor prescribes something and the insurance says "no," don't just walk away. My team and I will actually get on the phone with your doctor and the insurance company to find a version of that medicine that is covered.
Maximize Your "Extra Help"
Many people in our neighborhood qualify for "Extra Help" (also known as the Low-Income Subsidy) but haven't applied. This can lower your premiums and bring your copays down to just a few dollars. If you’re struggling to afford your monthly costs, stop by the pharmacy. We can’t sign you up ourselves, but we can point you to the right local Bronx resources to get your application started.

Does Medicaid pay for over-the-counter stuff like aspirin or vitamins?
